Services for Individuals
Your Care Partners Individual Program provides resources and service coordination for individuals and their caregivers in the midst of an eldercare crisis or who are planning for the future. Our program includes several features designed to help alleviate some of the stress and anxiety associated with finding quality eldercare resources and services including:
- A comprehensive Needs Assessment and Personalized Care Plan with objective, unbiased recommendations for services and providers
- Assistance with preparation for meetings or discussions with professional service providers and with scheduling appointments when needed
- Discounts from our service providers (where available)
- Phone and/or email access to a qualified Care Partner
